Measuring body fat In view of the limitations of the BMI a formal assessment of obesity should include an actual measurement of body trends body fat whenever possible. This is particularly important during the course of a weight loss programme as changes in BMI will simply reflect the change in body weight since adult height will remain constant. To measure the relative proportion of fat and lean tissue which is lost requires a direct measurement of body composition. The most accurate methods to measure body fat are difficult or expensive or both! The classical method involves calculating body density. This requires a measurement of body weight and body volume. Weight is easily measured, but volume is more complex. Body volume is usually measured by water displacement (Archimedes principle), which involves totally submerging the subjects underwater. |

| The body mass index can also be used in children, however the cut-offs for children are different from those for adults and change significantly with age. Body mass index, WHO classification, Clinical classification 18.5 - 24.9 kg/m2, Healthy weight, Healthy 25.0 - 29.9 kg/m2, Grade 1 obesity, Overweight 3 30.0 - 39.9 kg/m2, Grade 2 obesity, 3 Obesity >40.0 kg/m2, Grade 3 obesity Morbid, obesity It 3 is, however, important to remember that the BMI only gives a measure of relative weight for height and not a direct measurement of fatness. In large groups of subjects there is a good relationship between excess weight and fatness, but individual subjects may have a relatively high BMI, yet remain lean. The classic example of misclassification using the BMI is a body builder, with a very well developed musculature. Here the excess weight, over and above reference figures, is composed predominantly of lean tissue rather than fat. At the other extreme, old people may have a normal or low BMI but have a high proportion of fat because lean tissue mass decreases with age. |
